A depressão na gravidez pode comprometer o bem-estar materno e o desenvolvimento infantil. O tratamento com antidepressivos é muitas vezes necessário, mas levanta preocupações devido a potenciais efeitos adversos, como parto prematuro, baixo peso ao nascer, alterações no desenvolvimento cognitivo, síndrome de abstinência neonatal, dificuldades na adaptação perinatal e impacto no temperamento infantil. Esta dissertação analisa os benefícios e riscos do uso de antidepressivos na gravidez, abordando a epidemiologia da depressão perinatal, as principais classes de antidepressivos e os seus efeitos. Além disso, discute o papel do acompanhamento farmacêutico na gestão do tratamento. Os inibidores seletivos da recaptação da serotonina (ISRSs) são os mais prescritos na gravidez devido ao seu perfil de segurança relativamente favorável. No entanto, a sua utilização continua a exigir uma avaliação cuidadosa, dada a possibilidade de efeitos adversos. A decisão de manter ou interromper o tratamento deve ser individualizada, considerando a gravidade da depressão e os riscos associados à exposição aos fármacos. O estudo reforça a necessidade de mais investigações sobre a segurança dos antidepressivos na gravidez e destaca a importância de alternativas terapêuticas, como a psicoterapia e o suporte social, priorizando sempre a saúde mental materna.
Depression during pregnancy can compromise maternal well-being and child development. Treatment with antidepressants is often necessary, but it raises concerns due to potential adverse effects, such as preterm birth, low birth weight, changes in cognitive development, neonatal withdrawal syndrome, difficulties in perinatal adaptation, and impact on infant temperament. This dissertation analyzes the benefits and risks of using antidepressants during pregnancy, addressing the epidemiology of perinatal depression, the main classes of antidepressants and their effects. In addition, it discusses the role of pharmaceutical care in managing the treatment. Selective serotonin reuptake inhibitors (SSRIs) are the most prescribed during pregnancy due to their relatively favorable safety profile. However, their use still requires careful evaluation, given the possibility of adverse effects. The decision to maintain or discontinue treatment should be individualized, considering the severity of depression and the risks associated with exposure to the drugs. The study reinforces the need for further research on the safety of antidepressants in pregnancy and highlights the importance of therapeutic alternatives, such as psychotherapy and social support, always prioritizing maternal mental health.
