Benefícios da medicina dentária para a saúde sistémica e sua integração no Serviço Nacional de Saúde: revisão narrativa

Introdução: A Medicina Dentária tem-se vindo a afirmar, cada vez mais, como uma área do conhecimento científico aplicado à saúde com repercussões recíprocas diretas e indiretas sobre a sua vertente sistémica no ser humano, daí a gradual, ainda que morosa, incorporação nos vários sistemas de saúde, um pouco por todo o mundo.
Objetivo: A presente dissertação tem como objetivo identificar e analisar os benefícios da integração da Medicina Dentária no Serviço Nacional de Saúde, na especificidade de cada uma das suas especialidades reconhecidas, considerando as existentes diversas tipologias e regimes das instituições no contexto português. Perspectiva ainda, reunir informação acerca de outras realidades da Medicina Dentária externas a Portugal, nomeadamente a da União Europeia, a do Brasil, a de Inglaterra e a dos Estados Unidos da América, em relação à sua integração nos respectivos sistemas de saúde, como quais as especialidades reconhecidas nessas realidades do globo.
Metodologia: No sentido de alcançar os objetivos deste trabalho, realizou-se uma revisão narrativa da literatura, considerando qualquer tipologia de artigos redigidos na língua portuguesa e língua inglesa, como qualquer outro documento emanado por Entidade ou autoridade reconhecidas pelo Estado Português, nomeadamente legislação em vigor até à data. Determinou-se ainda a exclusão de qualquer documento ou produção científica que não se encontrasse redigido na língua portuguesa ou língua inglesa, tal como, se excluíram documentos já sem aplicabilidade legal no contexto português, quer seja por revogação, quer por legislação que se tenha, entretanto, sobreposto ou tenha anulado alguma anterior. Quanto ao aspecto temporal, estabeleceu-se a pesquisa até ao limite dos últimos 5 anos, no período compreendido entre 2019 e 2025. A formulação da questão desenvolveu-se sob a forma do acrónimo PICo, que é: População (P): Medicina Dentária; Intervenção (I): Integração e organização; Contexto (Co): Serviço Nacional de Saúde.
Conclusão: Apesar de a evidência científica existente indicar a relação de reciprocidade entre a saúde oral e a saúde sistémica, não parece ter sido a suficiente para convencer a classe política a privilegiar políticas sólidas de integração de serviços de saúde oral no sector público português. Concluiu-se que existe uma significativa disparidade entre as realidades portuguesa, europeia, brasileira, inglesa e norte-americana, tanto nas especialidades estatalmente reconhecidas, tal como, na integração de serviços de saúde oral nos vários sistemas de saúde.
Introduction: Dental Medicine has been progressively establishing itself as a area of scientific knowledge applied to health, with direct and indirect reciprocal repercussions on its systemic aspect in the human being. Hence, its gradual, albeit slow, incorporation into the various Health Systems worldwide. Objective: This dissertation aims to list and explain the benefits of integrating dental medicine into the Serviço Nacional de Saúde, specifically for each of its recognized specialties, considering the various typologies and systems established in the Portuguese context. It also aims to collect information about other international realities of dental medicine, namely those in the European Union, Brazil, England and the United States of America, regarding its integration into their respective health systems, such as the recognition of their respective specialties. Methodology: To achieve the objectives of this study, a narrative review of the literature was conducted, considering any type of study written in Portuguese or English Languages, as well as any other document issued by an entity or authority recognized by the Portuguese State, including legislation in force to date. We also excluded any document or scientific production not written in Portuguese or English Languages, as well as documents no longer applicable in the Portuguese context, whether due to revocation or legislation that has since superseded or annulled previous legislation. Regarding the time frame, the research was established up to the last five years, in the period between 2019 and 2025. The formulation of the question was developed in the form of the acronym PICo, which is:Population (P): Dental Medicine; Intervention (I): Integration and organization; Context (Co): Serviço Nacional de Saúde. Conclusion: Although existing scientific evidence indicates a reciprocal relationship between oral health and systemic health, it does not appear to have been sufficient to convince the political class to prioritize solid policies for the integration of oral health services in the portuguese public sector. The conclusion is that there is a significant disparity between the Portuguese, European, Brazilian, English, and North American realities, both in state-recognized specialties and in the integration of dentistry into the various health systems.
