Abstract(s)
Introdução: A rotura aguda do tendão de Aquiles (TA) apresenta uma elevada taxa lesiva, e sua etiopatogenia é multifatorial. Uma das opções terapêuticas é a abordagem cirúrgica, seguida de uma reabilitação pós-cirúrgica do tipo convencional ou acelerada. Objetivo: Analisar os outcomes da reabilitação acelerada em adultos após cirurgia de uma rotura aguda do tendão de Aquiles. Metodologia: foi realizada uma pesquisa na base de dados Pubmed, Science Direct, PEDro e Web of Science. Após a seleção dos estudos, seguindo os critérios de inclusão e exclusão, foi efetuada uma análise de qualidade metodológica pelo checklist da Critical Appraisal Skills Programme (CASP). Resultados: Foram selecionados 6 estudos, incorporando 290 pacientes sujeitos a cirurgia após rotura aguda. Apesar da diversidade dos tipos de cirurgia e dos protocolos implementados, a reabilitação acelerada proporcionou incrementos significativos no espaço de 13 meses no ganho de amplitude de movimento e de força muscular, na diminuição da dor e da atrofia, na capacidade de elevação do calcanhar, no tempo de retorno à vida profissional, e na redução das complicações dérmicas e tendinosas. Conclusão: A integração de uma reabilitação acelerada após cirurgia de uma rotura aguda potencia a recuperação do tendão, tanto a nível histológico, funcional, como sintomatológico.
Background: The acute Achilles tendon rupture has a high lesion rate and its etiopathogenesis is multifactorial. One of the therapeutic options is the surgical approach, followed by a conventional or accelerated post-surgical rehabilitation. Objectives: Analyze the outcomes of an accelerated rehabilitation after surgery of an acute Achilles tendon rupture. Methodology: A search was performed in the database Pubmed, Science Direct, PEDro and Web of Science. After the selection of the studies, following the inclusion and exclusion criteria, a methodological quality analysis was performed by the checklist of the Critical Appraisal Skills Program (CASP). Results: Six studies were selected, incorporating 290 patients who underwent surgery after acute rupture. Despite of the diversity of the types of surgery and the protocols implemented, accelerated rehabilitation provided significant increments in the 13-month span in the range of motion and muscle strength gain, the decrease in pain and atrophy, the heel lifting capacity, the time to return to work, and reduction of dermal and tendon complications. Conclusion: The integration of an accelerated rehabilitation after surgery of an acute rupture enhances the recovery of the tendon, both histologically, functionally and symptomatically.
