A cópia da Figura Complexa de Rey na avaliação do funcionamento executivo: um estudo exploratório com estudantes universitários

A literatura especializada tem vindo a sublinhar o papel da Figura Complexa de Rey na avaliação neuropsicológica do funcionamento executivo, tendo-se assistido ao desenvolvimento de sistemas de cotação qualitativa com este fim. A inexistência de um sistema consentâneo, por um lado, e a escassez de estudos psicométricos e normativos, em particular no contexto português, motivou a realização do presente trabalho. Concretamente, pretendeu-se avaliar o funcionamento executivo em adultos saudáveis através desta figura, com a finalidade de caracterizar as principais estratégias de organização e de planeamento utilizadas na sua cópia e de analisar a aplicação do sistema de cotação de Savage-Deckersbach na avaliação dessas estratégias. Os 34 estudantes universitários que integraram a amostra foram avaliados através da Figura Complexa de Rey, do Teste de Stroop, do Teste de Trilhas, do Teste de Fluência Ortográfica e da Torre de Hanói. Os resultados indicaram uma reduzida variabilidade de desempenho na Figura Complexa de Rey, traduzindo estratégias de organização muito similares, sendo a cópia maioritariamente iniciada pelo retângulo base e terminada pelo vértice do triângulo. Foi ainda observada a inexistência de correlação entre o índice de Organização do sistema de Savage-Deckersbach e os testes de Stroop, parte A do teste das Trilhas, Fluência Ortográfica e Torre de Hanói, o que sugere que o controlo inibitório e a velocidade de processamento (presentes nestas tarefas) terão um menor envolvimento na cópia da figura. A única correlação significativa encontrada ocorreu entre o índice de Organização e as pontuações do teste das Trilhas (parte B e score B-A), pontuações estas que refletem uma medida global do funcionamento executivo. No seu conjunto, os resultados afiguram-se promissores quanto às potencialidades do sistema de cotação de Savage-Deckersbach na avaliação clínica do funcionamento executivo de adultos.
The role of the Rey Complex Figure on the neuropsychological assessment of executive functioning has been highlighted in the specialized literature, with the development of qualitative scoring systems for this purpose. The lack of a consensual system and the scarcity of psychometric and normative studies, particularly in the Portuguese context, motivated the conduction of the present study. Specifically, it was intended to assess executive functions in healthy adults through this figure, in order to characterize the main organizational and planning strategies used in its copy and to analyse the validity of the Savage-Deckersbach scoring system in the evaluation of these strategies. The 34 university students who participated in this study were evaluated through the Rey Complex Figure, the Stroop Test, the Trail Making Test, the Orthographic Fluency Test and the Hanoi Tower. Results showed reduced variability of performance in the Rey Complex Figure, suggesting very similar organizational strategies, with the copy beginning mostly initiated by the base rectangle and finished by the vertex of the triangle. The inexistence of correlation between the Organizational index of the Savage-Deckersbach system and the Stroop test, Part A of the Trail Making Test, Orthographic Fluency and the Hanoi Tower was also observed, suggesting that inhibitory control and processing speed (both present in these tasks) have a lower involvement in the copy of the figure. The only significant correlation found occurred between the Organizational index and part B and score B-A of the Trail Making Test, reflecting a global measure of executive functioning. Overall the results are promising, regarding the potential of the Savage-Deckersbach scoring system in the clinical assessment of executive functioning in adults.
