Abstract(s)
A lombalgia crónica (LC) é uma condição prevalente, dispendiosa e desafiadora na prática clínica do fisioterapeuta. No tratamento desta condição, o fisioterapeuta deve ter como base a Prática Baseada na Evidência (PBE). De forma a sintetizar a informação existente na literatura, as Guidelines de Prática Clínica (GPC) fazem recomendações com base na evidência disponível acerca das intervenções mais eficazes, minimizando o tempo e o custo da intervenção. Objetivo: Realizar uma revisão de literatura, de modo a identificar as intervenções do fisioterapeuta na LC através da PBE, segundo recomendações de GPC. Metodologia: Pesquisa nas bases de dados da PubMed e PEDro, para identificar GPC que abordem o uso de modalidades relacionadas com o fisioterapeuta na intervenção na LC. Critérios de inclusão: Guidelines publicadas com um limite temporal de 10 anos (2006 e 2016), escritas em Inglês e com livre acesso na íntegra. Critérios de exclusão: artigos que fossem estudos experimentais, estudos de revisão, revisão de Guidelines e Guidelines que não abordassem a utilização de modalidades fisioterapêuticas na intervenção na LC. Resultados: A presente revisão da literatura inclui uma totalidade de 8 Guidelines. Para além de modalidades relacionadas com a terapia manual, a termofototerapia e modalidades cinesiológicas, que estão diretamente relacionadas com a intervenção do fisioterapeuta na LC, são ainda abordadas outras terapêuticas, como o yoga, a acupuntura, o uso de ortóteses e a intervenção multidisplinar; modalidades farmacológicas orais e invasivas e procedimentos cirúrgicos. Conclusão: Com base nas guidelines revistas, o fisioterapeuta deverá intervir na LC através do exercício terapêutico e da implementação de estratégias / aconselhamentos que promovam a autonomia do paciente para o tratamento desta condição. Deve incluir também a terapia manual na sua intervenção. Contudo, esta modalidade deverá ser utilizada a curto prazo e como complemento a um programa de exercício terapêutico.
Chronic low back pain (CLBP) is a prevalent, expensive and challenging condition in the physiotherapist's clinical practice. In the treatment of this condition, the physical therapist should be based on the Evidence Based Practice (EBP). In order to synthesize existing information in the literature, the Clinical Practice Guidelines (GPC) make recommendations based on available evidence about the most effective interventions, minimizing the time and cost of intervention. Objetive: Conduct a systematic review in order to identify the physiotherapist interventions in the CLBP through the EBP, according to recommendations of GPC. Methods: Research in PubMed and PEDro databases, in order to identify CPG that address the use of modalities related to the physiotherapist intervention in CLBP. Inclusion criteria: Guidelines published with a time limit of 10 years (2006 and 2016), written in English and with free full text. Exclusion criteria: articles that were experimental studies, review studies, revision of Guidelines and Guidelines that didn’t address the use of physiotherapeutic modalities in intervention in Chronic Low Back Pain. Results: The present literature review includes a total of 8 Guidelines. In addition to modalities related to manual therapy, thermo-therapy and kinesiological modalities, which are directly related to the physiotherapist's intervention in CL, other therapies, such as yoga, acupuncture, the use of orthoses and the multidisciplinary intervention, are also addressed; Oral and invasive pharmacological modalities and surgical procedures. Conclusion: Based on the revised guidelines, the physiotherapist should intervene in CL through therapeutic exercise and the implementation of strategies / counseling that promote the autonomy of the patient to treat this condition. The physiotherapist also has manual therapy that should be used as a complement to a program of therapeutic exercise and for a short-term.
