Os implantes em titânio vêm sendo cada vez mais utilizados em medicina dentária como pilares na reabilitação protética tanto em desdentados parciais como em pacientes totalmente edêntulos. Esta crescente utilização de implantes resulta fundamentalmente do facto de que inúmeros trabalhos de investigação levados a efeito em vários países terem provado a sua eficácia e fiabilidade mesmo quando considerada a longo prazo. Inicialmente era recomendado um certo tempo de espera pela colocação em carga após a cirurgia, sem prótese ou com prótese removível. Durante este tempo havia claro prejuízo para o paciente em funções tão importantes como a mastigação, a dicção, e a estética. Nos últimos anos, vários investigadores têm estudado a viabilidade de carga imediata sobre implantes em inúmeras situações clínicas. No presente trabalho é feita uma revisão bibliográfica, tão aprofundada quanto possível, sobre este tema que se mantém inteiramente actual e que não deixa de suscitar alguma controvérsia. Começa-se por apresentar em traços muito gerais uma breve história da implantologia. Em seguida, é abordado o problema da estabilidade implantar sobretudo do ponto de vista das principais métodos utilizados na sua avaliação e dos métodos de quantificação e qualificação óssea. São analisados os principais factores que influenciam o sucesso dos implantes; factores relacionados com a cirurgia, com o hospedeiro, com o implante e com a oclusão. Passa-se depois à introdução do conceito de carga imediata e à sua abordagem numa perspectiva histórica considerando a crescente utilização deste procedimento clínico em implantologia. Finalmente, é abordada a problemática da colocação de implantes imediatos em pacientes comprometidos. Conclui-se com a apresentação de algumas conclusões e da lista de referências bibliográficas que sustentam este trabalho.
Titanium implants are being used increasingly in prosthetic rehabilitation as pillars in partially or totally edentulous patients. This increasing use of implants results basically of the fact that innumerable works of investigations had been taken in some countries that have proven its effectiveness and reliability even when considered in the long term. Initially it was recommended a certain free loading time for the placed implants after surgery, with or without removable prosthesis. During this time, it clearly had damage for the patient in so important functions as the chew and the speech, and even it the aesthetic level. In the last years, some investigators have studied the immediate load viability on implants in innumerable clinical situations. In the present work a bibliographical revision is made, so deepened as possible, on this subject that is entirely actual and with some controversy. One brief history of the implantology is started for presenting. After that the problem of the stability is boarded to implant over all of the point of view of the main methods used in its evaluation and of the main factors that influence the success of the implantations. Introduction of the concept of immediate load and to its boarding in a historical perspective is transferred later a considering the increasing use of this clinical procedure in implantology. Also the problematic one of the rank of immediate implantations in engaged patients is boarded. It is concluded with the presentation of some conclusions and the list of bibliographical references that support this work.
