Reabsorção radicular no movimento ortodôntico: tratamento com alinhadores vs tratamento com sistema multibracket – revisão sistemática

| dc.description.abstract | Introdução: O tratamento ortodôntico apresenta vários benefícios, mas também tem riscos que lhe são indissociáveis, onde se inclui a reabsorção radicular. A reabsorção radicular pode ser diagnosticada através de radiografias periapicais intraorais, ortopantomografias ou tomografia computadorizada de feixe cónico. Em comparação com os aparelhos ortodônticos fixos, os alinhadores são mais confortáveis, esteticamente agradáveis, higiénicos e menos dolorosos. Os alinhadores são muito diferentes dos aparelhos fixos em termos de fixação aos dentes e biomecânica; portanto, pode-se esperar diferenças na reabsorção radicular entre eles. Desta forma, pretendeu-se com este trabalho realizar uma revisão sistemática da literatura, visando investigar a incidência e a severidade da reabsorção radicular externa nos casos tratados com aparelhos ortodônticos fixos e nos casos tratados com alinhadores transparentes. Metodologia: Pretendeu-se responder à seguinte questão: Existem diferenças na reabsorção radicular após o tratamento ortodôntico com alinhadores comparativamente com o tratamento ortodôntico com sistema multibrackets?. Os critérios de elegibilidade para a elaboração desta revisão seguiram a estratégia PICOS: População – Pacientes com dentição permanente tratados ortodonticamente; Intervenção – Pacientes tratados ortodonticamente com alinhadores transparentes, com tempo de tratamento mínimo de 6 meses; Controlo – Pacientes tratados ortodonticamente com sistema multibracket, com tempo de tratamento mínimo de 6 meses; Outcome – Incidência, severidade e alteração nas dimensões radiculares antes e após o tratamento; Study design – Ensaios clínicos randomizados. A pesquisa foi realizada em diferentes bases de dados: PubMed, Web of Science e Lilacs, com os artigos elegíveis a serem identificados e selecionados por 2 revisores independentes, seguindo as guidelines PRISMA. A avaliação qualitativa da qualidade de cada estudo foi realizada através da aplicação da Consolidated Standards Of Reporting Trials (CONSORT) checklist. Resultados: Foram incluídos 8 artigos, todos eles com uma avaliação de qualidade metodológica entre moderada a alta. O período de follow-up médio variou entre 6 meses até 31 meses e foram comparadas as reabsorções radiculares. Os vários estudos analisaram a incidência e severidade da reabsorção radicular aplicando distintos métodos de avaliação. Na maioria dos estudos confirma-se que o grau de reabsorção radicular é de ligeira a moderada após o tratamento ortodôntico, tanto com os aparelhos fixos convencionais como com os alinhadores, sendo que os casos tratados com aparelho fixo em geral apresentaram reabsorção significativamente maior do que aqueles tratados com alinhadores. Conclusão: Esta revisão sugere que, embora os alinhadores possam não eliminar a reabsorção radicular durante o tratamento ortodôntico, a incidência e a gravidade são menores em comparação com o tratamento com os sistemas multibrackets. No entanto, a seleção do tratamento continua a ser um julgamento do médico-dentista com base em cada caso individual. | por |
| dc.description.abstract | Introduction: Orthodontic treatment has several benefits, but it also has risks that are inseparable from it, including root resorption. Root resorption can be diagnosed by intraoral periapical radiographs, orthopantomography, or cone beam CT scan. Compared to fixed braces, aligners are more comfortable, aesthetically pleasing, hygienic, and less painful. Aligners are very different from fixed braces in terms of fixation to the teeth and biomechanics; therefore, differences in root resorption can be expected between them. Thus, the aim of this study was to carry out a systematic review of the literature, aiming to investigate the incidence and severity of external root resorption in cases treated with fixed orthodontic appliances and in cases treated with clear aligners. Methodology: The aim was to answer the following question: Are there differences in root resorption after orthodontic treatment with aligners compared to orthodontic treatment with a multibracket system?. The eligibility criteria for the elaboration of this review followed the PICOS strategy: Population – Patients with permanent dentition treated orthodontically; Intervention – Patients treated orthodontically with clear aligners, with a minimum treatment time of 6 months; Control – Patients treated orthodontically with a multibracket system, with a minimum treatment time of 6 months; Outcome – Incidence, severity and change in root dimensions before and after treatment; Study design – Randomized controlled trials. The search was carried out in different databases: PubMed, Web of Science and Lilacs, with eligible articles to be identified and selected by 2 independent reviewers, following the PRISMA guidelines. The qualitative evaluation of the quality of each study was carried out through the application of the Consolidated Standards Of Reporting Trials (CONSORT) checklist. Results: 8 articles were included, all of them with a moderate to high methodological quality evaluation. The mean follow up period ranged from 6 months to 31 months and root resorption was compared. The various studies analyzed the incidence and severity of root resorption by applying different evaluation methods. In most studies, it is confirmed that the degree of root resorption is mild to moderate after orthodontic treatment, both with conventional fixed appliances and with aligners, and cases treated with fixed appliances generally showed significantly higher resorption than those treated with aligners. Conclusion: This review suggests that although aligners may not eliminate root resorption during orthodontic treatment, the incidence and severity are lower compared to treatment with multibracket systems. However, the selection of treatment remains a judgement of the dentist on a case-by-case basis. | eng |
