A Terapia Pulpar Vital (TPV) é um tratamento pulpar, com uma abordagem conserva-dora, em dentes com diagnóstico de polpa vital, restabelecendo a função normal do dente, sensibilidade e propriocepção. O correto diagnóstico pulpar, embora seja muitas vezes subjetivo, pois baseia-se em sinais e sintomas referidos pelo paciente, é determinante para a decisão clínica do médico dentista, quanto ao procedimento a realizar.
Este estudo propõe uma revisão integrativa da literatura publicada entre 2015 e 2025, que pretende facultar uma compreensão ampla acerca dos fatores que determinam a de-cisão clínica em efetuar esta terapia em dentes permanentes vitais ou tratamento endodôn-tico não cirúrgico convencional.
Esta revisão vai abordar questões sobre a eficácia da TPV, na evolução da técnica e dos materiais e em comparação a outros tratamentos alternativos, como o TENC, também vai procurar identificar os principais critérios clínicos, radiográficos assim como a taxa de sucesso a longo prazo.
A questão da pesquisa segue a estratégia PICO: População- Pacientes com pulpites re-versíveis/irreversíveis em dentes permanentes; Intervenção- Terapia Pulpar Vital; Com-paração- Comparação entre os diferentes materiais e com outros procedimentos alterna-tivos; Outcomes- Sucesso do tratamento, decisão e processo de decisão clínica pelo en-volvimento de fatores clínicos e individuais do paciente, complicações.
Vital Pulp Therapy (VPT) is a conservative pulp treatment aimed at teeth diagnosed with vital pulp, restoring the normal function of the tooth, sensitivity, and proprioception. Accurate pulp diagnosis, although often subjective as it relies on signs and symptoms reported by the patient, is crucial for the dental clinician’s decision-making regarding the appropriate treatment procedure. This study proposes an integrative literature review published between 2015 and 2025, intended to provide a comprehensive understanding of the factors influencing the clinical decision to perform VPT in permanent vital teeth versus conventional non-surgical endodontic treatment. The review will address questions concerning the efficacy of VPT, the evolution of the technique and materials, and comparisons with alternative treatments such as conventional non-surgical endodontic treatment (CNS-ET). It will also seek to identify the main clinical and radiographic criteria as well as the long-term success rates. The research question follows the PICO framework: Population – Patients with reversible or irreversible pulpitis in permanent teeth; Intervention – Vital Pulp Therapy; Comparison – Comparison between different materials and alternative procedures; Outcomes – Treatment success, clinical decision-making processes influenced by clinical and individual patient factors, complications; Study Design – Systematic literature reviews on the topic, observational studies, qualitative studies, and randomized clinical trials.
