This narrative review compare Orthodontic Extrusion versus Surgical Extrusion, that are used for the rehabilitation of teeth with compromised restaurability, evaluating both the advantages and the disadvantages to find out if one can be considered objectively better than the other. Orthodontic extrusion is recommended if highly predictable treatment is required, if an orthodontic device is already present and if it is necessary to preserve the vitality of the tooth or to treat teeth that are not comparable with an atraumatic extraction. Surgical extrusion is suggested for solving endodontic problems that cannot be treated with traditional orthograde endodontic techniques, or as an alternative to the extraction of teeth that cannot otherwise be saved. A combined therapy, that merges the advantages of both techniques, is proposed as a future therapy. Further studies about this combined therapy are needed.
