- Direct oral anticoagulants and oral procedures: an integrative reviewPublication . Azizi, Rashed; Begonha, Maria João; Moreira, TeresaIn order to eliminate some of the disadvantages of vitamin K antagonists, four direct oral anticoagulants (DOACs) dabigatran, rivaroxaban, apixaban e edoxaban, have been developed. Direct oral anticoagulants are used in atrial fibrillation or venous thromboembolism and have proven to be equally effective compared to warfarin. They don’t require monitoring and have less food and drugs interactions. However, there isn’t sufficient dates on the protocol to be applied in patients receiving DOACs undergoing dental surgery. The aim of this review is to evaluate the bleeding risk of patients taking direct oral anticoagulants who scheduled to undergo oral procedures. For that, a bibliographic search was made in databases PubMed, ScienceDirect and Scopus and following the PICO criteria and at the end of the selection process, 11 articles were found, which were included in this integrative review. The selected studies showed that the maintenance of DOACs during dental extractions appears to be a safe and effective approach when accompanied by adequate local bleeding control strategies. Discontinuation should be avoided in most cases due to the potential risk of thromboembolic events. Clinical decisions must be individualized, taking into account the type of dental procedure, patient comorbidities, and timing of the last anticoagulant dose. Institutional protocols, clinician training, and interdisciplinary communication remain essential pillars for the safe management of anticoagulated patients in dental care.
