Eficácia no movimento de intrusão: brackets vs Invisalign®. Revisão narrativa sistematizada

Introdução: O movimento de intrusão é um dos aspetos mais desafiadores e importantes no tratamento ortodôntico, utilizado essencialmente na correção de más oclusões e nivelamento do plano oclusal. Tradicionalmente, o movimento de intrusão tem sido alcançado com o aparelho fixo, os brackets convencionais, mas com os avanços tecnológicos, com o desenvolvimento dos alinhadores estéticos tipo Invisalign®, trouxeram novas opções de tratamento. No entanto, permanece a dúvida sobre qual destes sistemas oferece maior eficácia no controlo e previsibilidade do movimento de intrusão, especialmente em termos de conforto para o paciente e estabilidade dos resultados a longo prazo.
Objetivo: Analisar a eficácia do movimento de intrusão entre os sistemas de brackets convencionais e alinhadores estéticos tipo Invisalign®, de modo a contribuir significativamente para o campo da ortodontia contemporânea.
Metodologia: Foram realizadas pesquisas nas bases de dados eletrónica PubMed, B-On e ScienceDirect, além da consulta de livros de referência. Foram usados termos de pesquisa relacionados ao movimento de intrusão dentária, brackets e alinhadores estéticos do tipo Invisalign®, combinados com os marcadores booleanos “AND” e “OR”. Os critérios de inclusão incluíram o período temporal de 2013-2024. Após a seleção inicial, os artigos foram filtrados por título, resumo e leitura integral, considerando textos em inglês, português e italiano.
Discussão: Analisar e comparar a literatura disponível relativamente à eficácia do movimento de intrusão, abordando as vantagens e desvantagens de ambos os métodos, destacando-se que o Invisalign® é preferido por razões estéticas e de conforto, embora os brackets sejam mais eficazes para movimentos dentários complexos.
Conclusão: Após a análise dos artigos selecionados nesta pesquisa, chegou-se à conclusão que, até à data, o movimento de intrusão com alinhadores estéticos ainda tem limitações face ao tratamento convencional.
Introduction: The intrusion movement is one of the most challenging and important aspects of orthodontic treatment, used essentially to correct malocclusions and level the occlusal plane. Traditionally, the intrusion movement has been achieved with the fixed appliance, conventional brackets, but technological advances, with the development of Invisalign® type aesthetic aligners, have brought new treatment options. However, the question remains as to which of these systems offers greater efficacy in controlling and predicting intrusion movement, especially in terms of patient comfort and long-term stability of results. Aim: To analyse the effectiveness of intrusion movement between conventional bracket systems and Invisalign® type aesthetic aligners, in order to make a significant contribution to the field of contemporary orthodontics. Methodology: Searches were carried out in the electronic databases PubMed, B-On and ScienceDirect, as well as reference books. Search terms related to tooth intrusion movement, brackets and Invisalign®-type aesthetic aligners were used, combined with the Boolean markers ‘AND’ and ‘OR’. The inclusion criteria included the time period 2013-2024. After the initial selection, the articles were filtered by title, abstract and full reading, considering texts in English, Portuguese and Italian. Discussion: To analyse and compare the available literature regarding the effectiveness of intrusion movement, addressing the advantages and disadvantages of both methods, highlighting that Invisalign® is preferred for aesthetic and comfort reasons, although brackets are more effective for complex tooth movements. Conclusion: After analysing the articles selected in this research, we came to the conclusion that, to date, intrusion movement with aesthetic aligners still has limitations compared to conventional treatment.
