Abstract(s)
As doenças crónicas não transmissíveis têm aumentado substancialmente ao longo dos
últimos anos como resultado do aumento da população idosa e da esperança média de vida,
contribuindo para elevadas taxas de mortalidade e morbilidade ao nível mundial.
A insuficiência crónica renal é uma das doenças que mais problemas acarreta ao nível
terapêutico em virtude das comorbilidades associadas e à necessidade de implementar um
regime de politerapia.
O Farmacêutico, enquanto detentor de um conhecimento privilegiado das etapas de
farmacocinética, a qual está geralmente alterada nestes doentes, pode constituir uma
ferramenta eficaz nos cuidados e prevenção e acompanhamento dos transtornos causados
pela insuficiência renal crónica, melhorando o prognóstico e a qualidade de vida dos
portadores desta doença.
A inclusão destes profissionais em equipas multidisciplinares que acompanhem o
doente renal crónico, quer ao nível comunitário, quer ao nível hospitalar, pode ser um
modelo de saúde a seguir, se se verificar existirem alterações significativas nos índices de
mortalidade e morbilidade associados com gestões terapêuticas.
Noncommunicable chronic diseases have increased substantially over the past few years as a result of an increase of the elderly population and average life expectancy, contributing to high rates of mortality and morbidity worldwide. Chronic kidney disease is a disease that brings many problems to the therapeutic level due to the associated comorbidities and the polytherapy regimen. The Pharmacist owns a privileged knowledge of the pharmacokinetics steps, which are often altered in these patients, though being an effective tool in the care, prevention and monitoring of disorders caused by chronic kidney disease, improving the prognosis and the quality of life of patients. The inclusion of these professionals in multidisciplinary teams that accompany the patients with chronic kidney disease, whether at community level or in hospital, can be a role model of health, if there are significant changes in mortality and morbidity rates associated with therapeutic managements.
