Introdução: O envelhecimento é diferencial e uma constante cascata de alterações fisiológicas, sendo necessário analisar essas mudanças, tendo em conta a sua influência no estado nutricional da pessoa idosa. O “paradoxo da obesidade” é um conceito relacionado com o estado nutricional de indivíduos idosos que se refere à hipótese de aqueles com sobrepeso e obesidade apresentarem melhor sobrevida do que os com peso normal.
Objetivo: Refletir sobre a avaliação do estado nutricional na pessoa idosa, de forma a abordar os diferentes pontos de corte do índice de massa corporal presentes na literatura, bem como o paradoxo da obesidade. Metodologia: A pesquisa bibliográfica foi conseguida através da Pubmed e do Google Académico, utilizando palavras-chave específicas. Resultados: Os pontos de corte de índice de massa corporal da Organização Mundial de Saúde são os mais utilizados, no entanto há autores e grupos de trabalho que apresentam pontos de corte diferentes e específicos para a população idosa, com base no paradoxo da obesidade. Conclusões: Existe a hipótese de valores mais elevados de índice de massa corporal poderem ser protetores em idosos com determinadas patologias, mas esta associação poderá estar sujeita a confundimento, a bibliografia é escassa e, por isso, é necessário uma maior investigação nesta área.
Introduction: Aging is differential and a constant cascade of physiological changes, and it is necessary to analyze these changes, taking into account their influence on the nutritional status of the elderly person.The “obesity paradox” is a concept related to the nutritional status of elderly individuals that refers to the hypothesis that those with overweight and obesity have better survival that those with normal weight. Objective: To reflect on the assessment of nutritional status in elderly people, in order to address the different body mass index cutoff points present in the literature, as well as the obesity paradox. Methodology: The bibliographic search was carried out through Pubmed and Google Academics, using specific keywords. Results: The World Health Organization's body mass index cutoff points are the most used, however there are authors and working groups that present different and specific cutoff points for the elderly population, based on the obesity paradox. Conclusions: There is a hypothesis that higher values of body mass index may be protective in elderly people with certain pathologies, but this association may be subject to confusion, the literature is scarce and, therefore, further research in this area is necessary.
