A instalação de implantes e de próteses implanto-suportadas são atualmente consideradas procedimentos básicos nas práticas odontológicas em todo o mundo. Um dos fatores considerados fundamentais para o sucesso a longo prazo em reabilitações implanto-suportadas diz respeito à correta seleção do sistema de retenção e do tipo de componente entre o implante e a prótese. Entretanto, considerando a ampla gama de componentes protéticos sobre os implantes osseointegráveis disponíveis, a correta seleção do pilar intermediário nas mais variadas situações clínicas torna-se um desafio. Este trabalho apresenta e discute os principais critérios a serem avaliados durante a seleção dos pilares intermediários para próteses implanto-suportadas, dentre os quais: o tipo de conexão e plataforma protética utilizados, a angulação do implante em relação aos dentes adjacentes, o espaço interoclusal existente, a profundidade do sulco, se a prótese é isolada ou múltipla, cimentada ou parafusada. Conclui-se que a seleção do pilar intermediário deve ser considerada ainda na fase de planejamento reverso, pois tanto o sistema de implante selecionado quanto os aspectos clínicos presentes influenciam na seleção correta do pilar.
The installation of implants and implant-supported prostheses are currently considered basic procedures in dental practices around the world. One of the factors considered to be fundamental for long-term success in implants-supported rehabilitations concerns the correct selection of the retention system and the type of abutment between the implant and the prostheses. However, considering the wide range of prosthetic components on osseointegratable implants available, the correct selection of the abutment becomes a challenge. This paper presents and discusses the main criteria to be evaluated during the abutment selection for implant-supported prostheses, among which: the type of connection and prosthetic platform used, the angulation of the implant in relation to the adjacent teeth, the interocclusal space, the peri-implant sulcular depth, single or multiple prosthesis, screwed or cemented prostheses.. It is concluded that the abutment selection should be considered during the reverse planning phase, since both the implant system selected and the clinical aspects influence the correct abutment selection.
