Introdução: O tratamento ortodôntico visa corrigir desalinhamentos dentários e disfunções oclusais, através do uso de aparelhos, que promovem o movimento gradual dos dentes para as suas posições corretas. O movimento de expansão, utilizado para aumentar o diâmetro da arcada dentária e criar mais espaço entre os dentes, pode ser alcançado com aparelhos fixos, removíveis ou alinhadores, visando melhorar o alinhamento e a oclusão dentária para um sorriso funcional e estético. Com o surgimento dos alinhadores, houve uma alternativa estética e confortável ao tratamento convencional, apesar de haver debates sobre a sua eficácia. Objetivo: Analisar a eficácia do movimento de expansão, com tratamento ortodôntico convencional vs sistema Invisalign, comparando as vantagens e desvantagens e resultados finais entre ambos os sistemas. Metodologia: Foi efetuada uma pesquisa bibliográfica, através da base de dados eletrónica MedLine, assim como no portal PubMed, ScienceDirect, B-on e Google Scholar. A pesquisa foi também complementada com livros de referência sobre o tema. O limite temporal foi de 10 anos, entre 2013 a 2023. Os critérios de inclusão foram: artigos com foco específico na eficácia comparativa entre alinhadores invisíveis e brackets, estudos realizados em dentição permanente e estudos realizados com a tecnologia Invisalign e tecnologia convencional. Aplicando-se estes critérios de inclusão e exclusão após a leitura do título e do resumo, foram selecionados 52 artigos para o desenvolvimento e 11 artigos para a meta-análise.
Conclusão: O Invisalign demonstra eficácia em casos de expansão moderada, especialmente em pré-molares e caninos, promovendo resultados satisfatórios, embora predominem movimentos dentoalveolares com leve inclinação dentária. Em comparação aos brackets tradicionais, que oferecem maior precisão em movimentos complexos, o Invisalign destaca-se pelo conforto e pela estética, sendo mais indicado para apinhamentos leves ou moderados. No entanto, seu uso requer um planeamento criterioso para assegurar resultados estáveis a longo prazo.
Introduction: Orthodontic treatment aims to correct dental misalignments and occlusal dysfunctions, through the use of appliances, which promote the gradual movement of teeth to their correct positions. The expansion movement, used to increase the diameter of the dental arch and create more space between the teeth, can be achieved with fixed, removable appliances or aligners, aiming to improve dental alignment and occlusion for a functional and aesthetic smile. With the emergence of aligners, there was an aesthetic and comfortable alternative to conventional treatment, despite there being debates about its effectiveness. Objective: To analyze the effectiveness of the expansion movement, with conventional orthodontic treatment vs Invisalign system, comparing the advantages and disadvantages and final results between both systems. Methodology: A bibliographic search was carried out through the MedLine electronic database, as well as on the PubMed, ScienceDirect, B-on, Google Scholar portals. The search was also complemented with reference books on the topic. The time limit was 10 years, between 2013 and 2023. The inclusion criteria were: articles with a specific focus on the comparative effectiveness between invisible aligners and brackets, studies carried out in permanent dentition and studies carried out with Invisalign technology and conventional technology. Applying these inclusion and exclusion criteria after reading the title and abstract, 52 articles were selected for development and 11 articles for meta-analysis. Conclusion: Invisalign demonstrates effectiveness in cases of moderate expansion, especially in premolars and canines, promoting satisfactory results, although dentoalveolar movements with slight tooth inclination predominate. Compared to traditional brackets, which offer greater precision in complex movements, Invisalign stands out for its comfort and aesthetics, being more suitable for mild or moderate crowding. However, its use requires careful planning to ensure stable long-term results.
