A presente revisão foi elaborada seguindo as diretrizes da PRISMA e teve como objetivo comparar os efeitos da terapia manual e do exercício de forma isolada em indivíduos com disfunções musculoesqueléticas da coluna avaliando dor,funcionalidade e custos socioeconomicos. Foram incluídos 18 ensaios clínicos randomizados que compararam de forma isolada ambas as intervenções. Estudos que combinavam as intervenções foram excluídos. A pesquisa foi realizada nas bases PubMed, PEDro, Web of Science e Google Académico. A qualidade metodológica foi avaliada através da escala PEDro. Ambos os tratamentos demonstraram-se efetivos com uma tendência favorável para a terapia manual na dor e funcionalidade. Apenas três estudos tiveram como foco os custos, o que não permite conclusões robustas. A heterogeneidade entre os estudos foi uma das principais limitações. Ambas as intervenções demonstraram-se eficazes e a escolha deve considerar perfil clínico do fisioterapeuta, as preferências do paciente e os recursos disponíveis.

Both treatments proved to be effective, with a favorable trend for manual therapy in pain and functionality. Only three studies focused on costs, which does not allow robust conclusions. Both interventions proved to be effective and the choice should consider the clinical profile of the physiotherapist, patient preferences and available resources.
