As disfunções temporomandibulares, são um grupo de patologias que afetam a articulação temporomandibular, músculos da mastigação e estruturas anexas, sendo uma das principais causas da dor orofacial. As DTMs intra-articulares quando associadas a dor e/ou limitação funcional reduzem a qualidade de vida dos pacientes. Esta revisão narrativa tem como objetivo descrever e comparar as indicações e sucesso terapêutico da artrocentese versus a artroscopia, para o tratamento de disfunções temporomandibulares intra-articulares. As duas técnicas demonstram resultados semelhantes, no que diz à melhoria da sintomatologia dolorosa e movimentos mandibulares. A artrocentese, menos invasiva, mais rápida e menos dispendiosa, deve ser a primeira opção de tratamento quando os tratamentos mais conservadores fracassam. Contudo quando a imagiologia colocar dúvidas quanto à patologia, a técnica artroscópica está indicada. Existe, no entanto, necessidade de mais estudos, com amostras maiores e um maior controlo das patologias a ser intervencionadas para validar as diferentes potencialidades das duas técnicas.
Temporomandibular disorders are a group of pathologies that affect the temporomandibular joint, chewing muscles and attached structures, being one of the main causes of orofacial pain. The intra-articular TMD when associated with pain and/or limited function reduce the quality of life of the patients. This literature review aims to describe and compare the indications and therapeutic success arthrocentesis versus arthroscopy for the treatment of intra-articular temporomandibular disorders. Both techniques show similar results regarding the improvement of painful symptoms and mandibular movements. Arthrocentesis, less invasive, faster and less expensive should be the first treatment option when more conservative treatments fail. However, when imaging questions the pathology, arthroscopic technique is indicated. However, there is a need for further studies, with larger samples and greater control of the pathologies being intervened to in order to validate the different potentials of the two techniques.
