A substituição de peças dentárias ausentes com recurso a implantes dentários é atualmente uma abordagem aceite e eficaz na restituição funcional e estética do sorriso. As deiscências de tecido mole peri-implantar são caracterizadas pela recessão da margem gengival ao redor da coroa implanto-suportada, com ou sem exposição da parte metálica do implante. Esta revisão sistemática tem como objetivo avaliar a eficácia das diferentes técnicas cirúrgicas utilizadas no tratamento de deiscências de tecido mole peri implantares. A pesquisa bibliográfica foi realizada nas bases de dados PubMed, ScienceDirect, EBSCO e B-on, seguindo critérios de inclusão previamente definidos. A triagem dos estudos foi realizada de acordo com as diretrizes PRISMA. Para a avaliação do risco de viés dos 11 estudos incluídos utilizou-se a ferramenta de avaliação crítica do Joanna Briggs Institute para estudos clínicos aleatorizados e estudos de caso. Os resultados indicam que, entre as diversas abordagens cirúrgicas analisadas, a técnica de retalho de avanço coronal conjugada com enxerto de tecido conjuntivo continua a ser a mais amplamente adotada, demonstrando resultados superiores em termos de recobrimento tecidual e melhoria estética. No entanto, conclui-se que há necessidade de estudos adicionais, especialmente ensaios clínicos aleatorizados, para determinar com maior rigor, a técnica mais eficaz para a correção de deiscências de tecido mole peri implantares.
Replacing missing teeth with dental implants is currently an accepted and effective approach to restoring the function and aesthetics of the smile. Peri-implant soft tissue dehiscence is characterized by recession of the gingival margin around the implant supported crown, with or without exposure of the metal part of the implant. This systematic review aims to evaluate the effectiveness of different surgical techniques used in the treatment of peri-implant soft tissue dehiscence. The bibliographic search was performed in the PubMed, ScienceDirect, EBSCO, and B-on databases, following previously defined inclusion criteria. The screening of studies was performed according to the PRISMA guidelines. The Joanna Briggs Institute critical appraisal tool for randomized clinical trials and case studies was used to assess the risk of bias in the 11 included studies. The results indicate that, among the various surgical approaches analyzed, the coronal advancement flap technique combined with connective tissue grafting remains the most widely adopted, demonstrating superior results in terms of tissue coverage and aesthetic improvement. However, it is concluded that further studies, especially randomized clinical trials, are needed to determine more rigorously the most effective technique for correcting peri-implant soft tissue dehiscence.
