Abstract(s)
A cirurgia apical endodôntica (CAE), ou microcirurgia endodôntica como é hoje chamada, tem sido bastante utilizada nos últimos anos, pois permite tratar um dente com lesão apical recorrente ou mesmo complementar uma intervenção recente quando a lesão não desaparece por completo com o procedimento endodôntico convencional. Desde que foi utilizada em 1756 por Pfaff e Berdmore, a CAE sofreu uma melhoria substancial, beneficiando das novas tecnologias aplicadas à Medicina Dentária, como é o caso da imagiologia digital e do microscópio cirúrgico; da Tomografia Computadorizada Cone Beam(TCCB); da cirurgia piezoelétrica em ossos, instrumentais ultra-sónicos e, especificamente à endodontia, o desenvolvimento de biomateriais de preenchimento de última geração, sendo Agregado Trioxido Mineral(MTA), Biodentina e cimento de Mistura Enriquecida de Cálcio(CEM) os exemplos mais emblemáticos.
Com este trabalho pretendo consolidar artigos pesquisados com informação específica relevante à CAE de forma a oferecer uma abordagem ampla, contemplando os meios de diagnóstico; o processo decisório do mesmo; o procedimento em si e os prognósticos esperados, baseando-nos, sobretudo, em estudos de caso envolvendo a CAE. Tenho a convicção de que este trabalho poderá contribuir para favorecer uma primeira abordagem ao tema da CAE nos seus aspetos mais relevantes.
The Endodontic Apical Surgery, or endodontic microsurgery as it is known today, has been vastly used in the past few years because it allows the treatment of a tooth with recurring apical lesion or even complement a recent intervention when a lesion does not disappear completely with the conventional endodontic procedure. Since it was used in 1756 by Pfaff and Berdmore, the Endodontic Apical Surgery has suffered substantial improvements, benefitting from new technologies applied to the Dentistry field, such as digital imageology and surgical microscope; Cone Beam Computerized Tomography(TCCB); piezoeletric surgery in bones, ultrasonic instruments and specifically to the endodontic field, the development of the latest generation of filling biomaterials such as Mineral Trioxide Aggregate(MTA), Biodentine and Calclium Enriched Mixture cement(CEM) being the most remarkable examples. With this paper I intend to consolidate researched articles with specific information relevant to the CAE as to offer a broader approach, contemplating the diagnostic methods; the decision process; the procedure itself and its expected results, based mostly in case reports in which the procedure was used. I believe that this paper may contribute to promote a first approach to the Endodontic Apical Surgery’s topic and its most relevant aspects.
