Objetivo: Comparar e avaliar a sensibilidade, especificidade e acurácia de novos testes e adaptações dos já existentes no diagnóstico de lesão do ligamento cruzado anterior (LCA). Metodologia: Pesquisa computadorizada nas bases de dados EBSCO, PubMed e Web of Science, utilizando a expressão: (accuracy OR specificity OR sensibility) AND (Orthopedic test OR clinical examination test OR diagnostic test) AND (ACL tear OR ACL injury OR anterior cruciate ligament injury OR anterior cruciate ligament tear), selecionando os estudos que abordavam a sensibilidade, especificidade e acurácia dos novos testes e testes adaptados no diagnóstico de lesões do LCA. Resultados: 7 estudos cumpriram os critérios de elegibilidade, apresentando 3 novas formas de diagnóstico: o Sinal de Lever, teste de Lachman em decúbito ventral e teste de gaveta anterior modificado, que demonstraram aptidão para diagnosticar a lesão do LCA. No entanto, a sua sensibilidade, especificidade e acurácia não são totalmente comparáveis com o teste de Lachman em decúbito dorsal, identificado na literatura como o melhor preditor clínico deste tipo de lesão. Conclusão: a evidência sugere que estes testes podem ser úteis no diagnóstico das lesões do LCA, quando associados aos testes padrão.
Aim: To compare and assess the sensitivity, specificity and accuracy of new tests and adaptations of the existing ones in the diagnosis of anterior cruciate ligament injury (ACL). Methodology: Computerized search in EBSCO, PubMed and Web of Science databases using the expression: (accuracy OR specificity OR sensibility) AND (Orthopedic test OR clinical examination test OR diagnostic test) AND (ACL tear OR ACL injury OR anterior cruciate ligament injury OR anterior cruciate ligament tear), selecting the studies that addressed the sensitivity, specificity and accuracy of new tests and adapted ones for the diagnosis of injury of the ACL. Results: 7 studies meet the eligibility criteria, introducing 3 new diagnostic tests: Lever´s sign, Lachman test in prone position and modified anterior drawer test, demonstrating aptitude for diagnosing ACL injury. However, their sensitivity, specificity and accuracy are not fully comparable with the Lachman test in supine position, identified in the literature as the best clinical predictor of this type of injury. Conclusion: evidence suggests that these tests can be useful in diagnosing ACL injuries, when combined with the standard ones.
