Abstract(s)
Introdução: durante o período gestacional o estiramento da musculatura abdominal pode causar uma separação dos feixes do músculo reto abdominal causando a formação da diástase do músculo reto abdominal (DMRA). A fisioterapia é fundamental no tratamento das mudanças corporais, promovendo a recuperação da tonicidade da musculatura abdominal através de vários métodos como Pilates, Eletroterapia, e a Cinesioterapia, na qual se inclui exercícios de fortalecimento abdominal, muitas vezes em conjunto com técnicas de exercícios respiratórios.
Objetivo: identificar qual a intervenção fisioterapêutica utilizada mais comumente no tratamento da diástase do músculo reto abdominal no período pós-parto, e quais os seus efeitos.
Metodologia: trata-se de uma revisão sistemática, fundamentada numa pesquisa computorizada de artigos científicos no mês de outubro de 2021, nas bases de dados PubMed, PEDro, LILACS e em outras fontes. A pesquisa teve como propósito identificar artigos que avaliassem o tipo e efeitos da intervenção fisioterapêutica utilizada no tratamento da DMRA no pós-parto. A qualidade metodológica foi analisada através Critical Appraisal Skills Programme (CASP).
Resultados: foram encontrados 293 artigos, dos quais 13 apresentavam características de elegibilidade. Dos estudos selecionados, 11 incluíram 691 mulheres puérperas com idades compreendidas entre 18 e 45 anos, os quais utilizaram diversas técnicas fisioterapêuticas. Os programas de exercícios, o fortalecimento do CORE, a ginástica abdominal hipopressiva, assim como o método de Pilates, e mesmo o uso do Tubigrip, foram efetivos no tratamento da DMRA. Já os resultados com a aplicação de Kinesiotaping não foram consensuais.
Conclusão: embora a intervenção fisioterapêutica utilizada nesta revisão ter sido, na sua maioria efetiva, os estudos referentes ao tratamento da diástase dos reto abdominais ainda são escassos, necessitando-se, assim, de mais programas fisioterapêuticos, com o intuito de tratar e/ou prevenir a diástase dos músculos reto abdominal em puérperas.
Introduction: during the gestational period, the stretching of the abdominal muscles may cause a separation of the rectus abdominal muscle bundles, causing the formation of diastasis of the rectus abdominis muscle (DRAM) and promoting the recovery of tonicity of the abdominal muscles through various methods such as Pilates, Electrotherapy, and Kinesiotherapy, which includes abdominal strengthening exercises, often in conjunction with breathing exercise techniques. Objective: to identify which physiotherapeutic interventions are most used in treating diastasis of the rectus abdominis muscle in the postpartum period and their effects. Methodology: This systematic review is based on a computerized search of scientific articles in October 2021 in the PubMed, Pedro, LILACS databases, and other sources. The investigation aimed to identify themes that evaluated the type and effects of the physiotherapeutic intervention used in DRAM treating in postpartum period. The methodological quality was analysed through the Critical Appraisal Skills Programme (CASP). Results: 293 articles were found, 13 of which presented eligibility characteristics. Of the selected studies, 11 included 691 postpartum women aged between 18 and 45 years, who used different physical therapy techniques. The exercise programs, strengthening of the CORE, hypopressive abdominal gymnastics, as well as the Pilates method, and even the use of Tubigrip, were effective in the DRAM treatment. On the other hand, the results with the application of Kinesiotaping were not consensual. Conclusion: although the physical therapy intervention used in this review was, for the most part, effective, the studies regarding the physiotherapist's role in treating rectus abdominis diastasis are still scarce, thus requiring more physiotherapeutic programs to treat or prevent diastasis of the rectus abdominis muscles in puerperal.
