Introdução: Os exercícios excêntricos têm sido amplamente utilizados no tratamento da epicondilite lateral, mas a sua eficácia foi recentemente questionada. Objetivo: analisar a evidência acerca do efeito dos exercícios de fortalecimento excêntrico no tratamento da EL. Metodologia: Foi realizada uma pesquisa computorizada, entre Janeiro e Abril de 2025, nas bases PubMed, PEDro e Web of Science, para identificar estudos randomizados controlados que avaliassem a eficácia dos exercícios excêntricos no tratamento da epicondilite lateral. A qualidade metodológica foi avaliada pela escala PEDro. Resultados: Foram incluídos sete ensaios clínicos randomizados (n = 447), com qualidade metodológica média de 5.9. O fortalecimento excêntrico demonstrou ganhos significativos na força muscular em 5 estudos, especialmente na preensão manual. Melhorias no nível de dor foram observadas em 3 estudos, e a função do membro superior apresentou progressos em 3 estudos. Conclusão: Os exercícios excêntricos têm potencial terapêutico na epicondilite lateral, sobretudo no fortalecimento muscular, mas a combinação com outras estratégias parece otimizar os resultados.
Introduction: Eccentric exercises have been widely used to treat lateral epicondylitis, but their effectiveness has recently been questioned. Aim: To analyze the evidence regarding the effect of eccentric strengthening exercises in the treatment of lateral epicondylitis. Methods: A computerized search was conducted between January and April 2025 in the PubMed, PEDro, and Web of Science databases to identify randomized controlled trials evaluating the effectiveness of eccentric exercises in the treatment of lateral epicondylitis. Methodological quality was assessed using the PEDro scale. Results: Seven randomized controlled trials (n = 447) were included, with an average methodological quality score of 5.9. Eccentric strengthening showed significant improvements in muscle strength in 5 studies, particularly in grip strength. Pain levels improved in 3 studies, and upper limb function showed progress in 3 studies. Conclusion: Eccentric exercises have therapeutic potential in lateral epicondylitis, especially for muscle strengthening, but combining them with other strategies appears to enhance outcomes.
