Abstract(s)
O insucesso no tratamento endodôntico pode ser devido à contaminação bacteriana de um canal durante o tratamento endodôntico ou entre sessões e persistência de microrganismos apesar dos procedimentos de desinfeção realizados corretamente. Várias opções estão disponíveis para os profissionais: retratamento ortógrado, cirurgia endodôntica ou extração do dente.
Objetivo: O objetivo desta revisão sistemática é estudar as principais soluções após uma falha no tratamento endodôntico, incluindo as suas causas, mas sobretudo como resolvê-la. Desenvolver os diferentes critérios que nos permitirão tomar a melhor decisão entre um retratamento ortógrado e uma cirurgia endodôntica a longo prazo, para o dente, e quais são os fatores que permitirão o sucesso ou insucesso do retratamento.
Metodologia: Foi realizada uma pesquisa bibliográfica sistemática através de diversas plataformas de publicação científica, incluindo PubMed, Google Scholar e Base Search. Este trabalho foi submetido a uma estratégia de pesquisa seguindo o método PICO, bem como critérios de inclusão e exclusão, que possibilitaram a seleção de estudos relacionados com o sucesso e insucesso do retratamento endodôntico e as diferentes técnicas.
Resultados: Foram apresentados aspetos numéricos das áreas de cuidados dentários e outros assuntos relacionados com a medicina dentária, bem como o tipo de estudos e características de qualquer estudo relevante. A falha no tratamento endodôntico foi utilizada para obter dados de prevalência. O retratamento ortógrado foi utilizado para obter dados de segurança, sendo permitidas seis referências: um estudo de coorte prospetivo, um estudo de coorte retrospetivo, um estudo de coorte e três estudos transversais. A cirurgia endodôntica a longo prazo permitiu mapear três referências: um estudo transversal, um estudo randomizado controlado e um estudo de coorte prospetivo.
Conclusões: De acordo com a literatura incorporada nesta revisão sistemática, existe uma preferência na prática e no sucesso para o retratamento ortógrado, não cirúrgico, entre as diferentes técnicas estudadas nos oito artigos analisados.
Failure in endodontic treatment can be due to bacterial contamination of a canal during the endodontic procedure or between sessions, and the persistence of microorganisms despite properly performed disinfection procedures. Several options are available to professionals: orthograde retreatment, endodontic surgery, or tooth extraction. Objective: The objective of this systematic review is to study the main solutions after failure in endodontic treatment, including their causes, but especially how to resolve them. It aims to develop the different criteria that will allow us to make the best decision between orthograde retreatment and endodontic surgery in the long term for the tooth, and to identify the factors that will determine the success or failure of the retreatment. Methodology: A systematic bibliographic search was conducted through various scientific publication platforms, including PubMed, Google Scholar, and Base Search. This work followed a search strategy using the PICO method, as well as inclusion and exclusion criteria, allowing the selection of studies related to the success and failure of endodontic retreatment and the different techniques. Results: Numerical aspects of dental care areas and other topics related to dental medicine were presented, as well as the type of studies and characteristics of any relevant studies. Endodontic treatment failure was used to obtain prevalence data. Orthograde treatment was used to obtain safety data, with six references allowed: one prospective cohort study, one retrospective cohort study, one cohort study, and three cross-sectional studies. Long-term endodontic surgery allowed for mapping three references: one cross-sectional study, one randomized controlled trial, and one prospective cohort study. Conclusions: According to the literature included in this systematic review, there is a preference in practice and success for non-surgical orthograde retreatment among the different techniques studied in the eight analyzed articles.
