O impacto da infeção por HPV no cancro oral: revisão sistemática

| dc.description.abstract | Objetivos: O cancro é, atualmente, uma das principais causas de morte no mundo, estimando-se que o número de casos venha a aumentar. O cancro oral é o sexto tipo de cancro mais comum no mundo. Nos últimos anos, tem-se vindo a aceitar a infeção por HPV como um dos principais fatores etiológicos do cancro oral. O presente estudo tem como objetivo avaliar a relação potencial da infeção por HPV e o cancro oral, e o seu significado biológico. Métodos: Foi desenvolvida uma pesquisa bibliográfica, aplicando palavras-chave estabelecidas, em vários motores de pesquisa científica. Após a utilização do método PICO, para classificar os artigos, e com recurso de outras meta-análises, 66 artigos integraram a presente análise estatística, utilizando o programa stata. Resultados: A prevalência de tumores da cabeça e pescoço HPV-positivos foi de 38%, tendo-se verificado que, especificamente na orofaringe, 50% dos tumores eram HPV-positivos. O HPV16 foi o genótipo mais frequentemente encontrado (0.71%, 95% CI= 0.61-0.80). Apresentar uma infeção por HPV16 representou um elevada probabilidade de desenvolvimento de tumor da cabeça e pescoço (RR= 8.39 , 95% CI= 5.01-14.05) e, pacientes com hábitos de risco, apresentaram menor risco de desenvolvimento de tumores HPV-positivos (RR= 1.46 , 95% CI= 0.86-2.47 Vs RR= 2.12 , 95% CI= 1.29-3.58). Conclusões: Os tumores HPV-positivos representam uma fração considerável dentro dos tumores da cabeça e pescoço e, especialmente, na região da orofaringe. Assim, torna-se fundamental, aquando do diagnóstico de um tumor oral, a avaliação da positividade do tumor para o HPV. | pt_PT |
| dc.description.abstract | Purpose: Currently, cancer is one of the leading causes of death in the world, and it is predicted that the number of cases will increase with time. Oral cancer is the sixth most common type of cancer. In recent years, HPV infection has been accepted as one of the main etiological factors of oral cancer. The present study aims to evaluate the impact of HPV infection in oral cancer. Methodology: A literary research was first conducted, applying the established keywords in several scientific research engines. After using the PICO method to classify the articles, and employing other meta-analyzes, 66 articles integrated the statistical analysis, completed through the stata programme. Results: The prevalence of HPV-positive head and neck cancers was 38%, and it was found that, specifically in the oropharynx, 50% of these tumors were HPV-positive tumors. HPV16 was the most frequently found genotype (0.71%, 95% CI = 0.61-0.80). The existence of HPV16 infection showed a high probability of developing head and neck tumors (RR = 8.39, 95% CI = 5.01-14.05), and patients with risk-consumption habits had a lower risk of developing HPV-positive tumors (RR = 1.46, 95% CI = 0.86-2.47 Vs RR = 2.12, 95% CI = 1.29-3.58). Conclusions: HPV-positive tumors represent a considerable fraction within head and neck cancers, and especially in the region of the oropharynx. Thus, the evaluation of tumor positivity for HPV is essential when diagnosing an oral tumor. | pt_PT |
