A doença celíaca é uma doença autoimune induzida pela ingestão do glúten em indivíduos geneticamente predispostos, diagnosticada mais frequentemente em crianças com sintomas gastrointestinais típicos, mas observa-se que muitos pacientes pediátricos apresentam a doença na forma assintomática. Esta revisão sistemática tem como principal objetivo informar os médicos dentistas sobre a sua importância no reconhecimento das manifestações orais da doença celíaca nos pacientes pediátricos, respondendo à seguinte questão: “O paciente pediátrico com doença celíaca apresenta maior prevalência de manifestações orais?”.
Desta forma, foi realizada uma pesquisa bibliográfica nas bases de dados PubMed, Scielo, Cochrane Library e Lilacs. Foram utilizadas combinações de palavras-chave MeSH com o marcador boleano “AND” e “OR”: “oral manifestations”, “celiac disease”, “pediatric dentistry”, “enamel defects”, “caries” e “oral aphthous”. Foram abrangidos estudos realizados em crianças com doença celíaca confirmada. Dos 241 artigos iniciais, foram incluídos 14 estudos que consideravam os objetivos propostos.
Verificou-se que as principais manifestações orais encontradas foram a estomatite aftosa recorrente e os defeitos de esmalte dentário. São ainda relatadas atraso na erupção dentária, queilite angular, glossodinia e xerostomia. Existem diversas manifestações orais na doença celíaca, que juntamente com a história clínica, o médico dentista pode ser a chave para diagnosticar formas silenciosas da doença celíaca.
Celiac disease is an autoimmune disease induced by gluten ingestion in genetically predisposed individuals, diagnosed more frequently in children with typical gastrointestinal symptoms, but it is observed that many pediatric patients present with the disease in the asymptomatic form. The main objective of this systematic review is to inform dentists about their importance in recognizing the oral manifestations of celiac disease in pediatric patients, answering the following question: "Do pediatric patients with celiac disease have a higher prevalence of oral manifestations?". A literature search was conducted in the PubMed, Scielo, Cochrane Library, and Lilacs databases. Combinations of MeSH keywords with the Boolean marker "AND" and "OR" were used: “oral manifestations”, “celiac disease”, “pediatric dentistry”, “enamel defects”, “caries” and “oral aphthous”. We included studies conducted in children with confirmed celiac disease. Of the initial 241 articles, 14 studies that met the proposed objectives were included. The main oral manifestations found are recurrent aphthous stomatitis and tooth enamel defects. Delayed tooth eruption, angular cheilitis, glossodynia, and xerostomia have also been reported. There are various oral manifestations in celiac disease, which, along with the clinical history, can enable the dentist to be key in diagnosing silent forms of celiac disease.
