A luxação da articulação temporomandibular (ATM) ocorre quando o côndilo mandibular se desloca para fora da fossa mandibular e não é capaz de retornar. As manifestações clinicas mais frequentes desta patologia são: dificuldade de fechar a boca, depressão pré-auricular, sialorréia, tensão dos músculos da mastigação e dor severa na região articular. A luxação pode ocorrer isoladamente ou em episódios repetitivos, sendo por isso chamada luxação espontânea ou recorrente. O tratamento da luxação da ATM divide-se em transitório, constituído por auto-redução e manobras de redução da luxação, e tratamento definitivo, que está dividido em tratamento conservador ou cirúrgico.
Este trabalho consiste numa revisão da literatura sobre a luxação da ATM, mencionando as manifestações clínicas e as diferentes abordagens no tratamento.
Esta revisão foi realizada com limite temporal, de 2000 a 2013, e utilizaram-se as seguintes palavras-chave: temporomandibular joint (TMJ) dislocation, TMJ recurrent luxation,treatment of TMJ dislocation, TMJ luxation surgery, TMJ disorders e TMJ anatomy.
São diversas as modalidades terapêuticas para tratamento da luxação da ATM. Os métodos conservadores resultam apenas no alívio temporário dos sintomas nos primeiros episódios de carácter agudo. Os tratamentos cirúrgicos apresentam melhores resultados nas situações recorrentes, por se tratar de um tratamento mais efetivo e definitivo. Dislocation of temporomandibular joint (TMJ) occurs when the mandibular condyle moves out of the mandibular fossa and isn´t able to return. The most frequent clinical manifestations of this pathology are: inability to close the mouth, depression in the preauricular area, drooling, masticatory muscles tension and severe pain in the joint. The dislocation may occur in isolation or in repetitive episodes, being so called spontaneous or recurrent dislocation. The treatment of dislocation of TMJ is divided into transient, including self-reduction and dislocation reduction maneuvers, and definitive treatment, which is divided into conservative or surgical treatment.
This work is a review of the literature on the dislocation of TMJ, mentioning the clinical manifestations and the different approaches in the treatment.
This review was performed with time limit, from 2000 to 2013, and used the following keywords: temporomandibular joint (TMJ) dislocation, TMJ recurrent luxation,treatment of TMJ dislocation, TMJ luxation surgery, TMJ disorders e TMJ anatomy.
There are many ways to treat the dislocation of TMJ. The conservative methods result in only temporary relief of symptoms in early episodes of acute nature. Surgical treatments have better results in recurring situations, because it is a more effective and definitive treatment.
