| dc.description.abstract | A cirurgia piezoelétrica constitui uma alternativa à cirurgia óssea convencional, permitindo a realização efetiva de osteotomias e osteoplastias nas mais variadas práticas cirúrgicas maxilo-faciais. O corte de osso é feito por meio da conversão da corrente elétrica em vibrações lineares ultrassónicas permitindo o corte de tecidos mineralizados com precisão micrométrica, preservando as estruturas moles adjacentes. Apesar de ter uma curva de aprendizagem maior em relação aos outros instrumentos rotatórios e oscilatórios, a possibilidade de um maior controlo cirúrgico e as vantagens pós-operatórias mostram o verdadeiro potencial desta técnica de corte por ultrassom. A perda parcial ou total de peças dentárias na região posterior da maxila leva à pneumatização do seio maxilar, dificultando assim a reabilitação com implantes. A cirurgia de levantamento do seio maxilar foi desenvolvida para ultrapassar este problema, permitindo a criação de um espaço a preencher com enxerto ósseo, aumentando a quantidade de osso e, por isso, permitindo a reabilitação oral. Por ser realizada numa área anatómica particularmente desafiante, o procedimento de levantamento do seio maxilar tem vindo a beneficiar com o uso do aparelho piezoelétrico, que por ter seletividade para os tecidos duros, evita uma das complicações mais commumente observadas na cirurgia oral, a perfuração da membrana de Schneider. A membrana sinusal pode ser perfurada aquando da preparação óssea, com instrumentos rotatórios tradicionais, ou no momento da divulsão da membrana, com elevadores manuais. A perda de integridade pode ocasionar complicações graves que podem comprometer não só a evolução do procedimento como também o período pós-cirúrgico. O instrumento piezo apresenta-se vantajoso na abordagem cirúrgica do seio maxilar, permitindo a manutenção da integridade da membrana de Schneider. | pt_PT |
| dc.description.abstract | Piezoelectric surgery is an alternative to conventional bone surgery, allowing the effective performance of osteotomies and osteoplasties in the most varied maxillofacial surgeries. The bone cutting is done by the conversion of electric current into linear ultrasonic vibrations allowing the cutting of mineralized tissues with micrometric precision, preserving the adjacent soft structures. Despite having a greater learning curve compared to other rotating and oscillatory instruments, the possibility of greater surgical control and the postoperative advantages show the true potential of this ultrasound cutting technique. The partial or total loss of dental pieces in the posterior region of the maxilla leads to pneumatization of the maxillary sinus, making it difficult to rehabilitate with implants. Surgery of the maxillary sinus was developed to overcome this problem, allowing the creation of a space to be filled with bone graft, increasing the amount of bone and, therefore, allowing oral rehabilitation. Because it is performed in a particularly challenging anatomical area, the sinus lift procedure has benefited from the use of the piezoelectric device, which, because of its selectivity to the hard tissues, avoids one of the most commonly complications observed in oral surgery, the perforation of Schneider’s membrane. The sinus membrane may be perforated during bone preparation, with traditional rotating instruments, or at the time of membrane divulsion with manual lifts. Loss of integrity can lead to serious complications that may compromise not only the evolution of the procedure but also the post-surgical period. The piezo instrument is advantageous in the surgical approach of the maxillary sinus, allowing the integrity of the Schneider’s membrane. | pt_PT |
