Diversos estudos apontam uma associação entre a periodontite e várias condições sistémicas. No contexto das doenças hepáticas, é crucial um maior esclarecimento.
Segundo a teoria da infeção focal, as bactérias, endotoxinas e mediadores inflamatórios locais podem penetrar na corrente sanguínea a partir das áreas inflamadas das gengivas, causando uma inflamação crónica de baixo grau e afetando órfãos distantes. O objetivo desta revisão narrativa é analisar as evidências atuais relativas à potencial associação entre a periodontite e as doenças hepáticas crónicas.
Foi realizada uma pesquisa em bases de dados com palavras-chave como “periodontite”; “doença hepática crónica”; “NAFLD”; “hepatite viral crónica”; “carcinoma hepatocelular”; “cirrose”; “stress oxidativo”; “tratamento periodontal”. Foram selecionados 74 artigos que atendiam aos critérios de inclusão e exclusão definidos.
A revisão da literatura permite compreender os mecanismos e as condições propícias ao desenvolvimento dessa associação. Confirma-se, assim, a relação entre periodontite e doenças hepáticas crónicas.
Several studies indicate an association between periodontitis and various systemic conditions. In the context of liver diseases, greater clarification is crucial. According to the focal infection theory, bacteria, endotoxins, and local inflammatory mediators can enter the bloodstream from inflame gum tissues, causing low-grade chronic inflammation and affecting distant organs. The aim of this narrative review is to analyze current evidence regarding the potential association between periodontitis and chronic liver diseases. A literature search using keywords as ‘periodontitis’, ‘chronic liver disease’, ‘NAFLED’, ‘chronic viral hepatitis’, ‘hepatocellular carcinoma’, ‘cirrhosis’, ‘oxidative stress’ and ‘periodontal treatment’ yielded a selection of 74 articles meeting inclusion and exclusion criteria. The literature review enables understanding of the mechanisms and conditions conducive to the development of this association, confirming the relationship between periodontitis and chronic liver diseases.
