Trauma oclusal e peri-implantite: revisão de casos clínicos

Esta revisão procurou avaliar o possível impacto que o trauma ou sobrecarga oclusal pode ter no aparelho peri-implantar e quanto e de que forma um reajuste ou correcção dos contactos oclusais com subsequente eliminação da sobrecarga pode ajudar a recuperar os tecidos peri-implantarios.
Foi realizada uma pesquisa sistematizada nas Bibliotecas Pubmed, B-on e Cochrane Library.
Os artigos que surgiram, com base nos temos de pesquisa utilizados, foram avaliados, analisados e selecionados e com base na seleção de artigos realizada foi elaborado um fluxograma PRISMA e a partir dos estudos resultantes a presente revisão foi construída.
A pesquisa de literatura foi limitada a estudos primários (Case Reports, Clinical Study e Clinical Trial), estudos humanos realizados nos últimos 10 anos, de língua Inglesa, que descrevam e avaliem trauma oclusal em implantes e de que forma ajustes oclusais nas restaurações sobre implantes podem ter relação com a incidência e, ou evolução de mucosite e perimplantite.
Os resultados evidenciam ausência de literatura significativa sobre o tema, existindo apenas alguns casos clínicos publicados que abordam numa perspetiva clínica a possível inter-relação entre trauma oclusal e alterações a nível dos tecidos peri-implantarios.
Este trabalho é assim de interesse científico e clínico porque a relação entre trauma oclusal e as complicações biológicas associadas aos tratamentos com implantes ainda não estão bem definidos, sendo de importância para a estabilidade e longevidade dos tratamentos, cada vez mais comuns, com implantes.
A presente revisão evidencia a necessidade da realização de novos protocolos de investigação e estudos bem estruturados adicionais, abordando a complexidade dos fatores relacionados à saúde peri-implantar. Futuros trabalhos de investigação são essenciais para o desenvolvimento de abordagens terapêuticas ainda mais eficazes e personalizadas, visando atender às necessidades específicas de cada paciente.
This review tried to assess the impact that occlusal trauma, or oclusal overload, can have on the peri-implant apparatus, and how much a readjustment or correction of occlusal contacts and elimination of occlusal stress can help restore the condition of the tissues surrounding the implants. A systemized search was conducted in Pubmed, B-on and Cochrane Library. The articles obtained, with search parameters used, were assessed, analyzed and selected. A PRISMA flowchart was compiled, and from the resulting studies, the review was constructed. The literature was limited to primary studies (Case Reports, Clinical Study and Clinical Trial), human studies conducted within the last 10 years, published in English, and studies describing occlusal trauma on implants and how occlusal adjustments on implant supported restorations may have a relationship with the incidence of mucositis and perimplantitis. The obtained results evidence the absence of relevant published literature about this topic, there are only a few published case reports that, from a clinical perspective, try to evaluate the relationship between oclusal trauma and changes to peri-implant tissues. This study is scientifically and clinically interesting because the relationship between occlusal trauma and biological complications associated with implant treatments is still not well defined, and it is important for the stability and longevity of implant treatments that are nowadays very common. The present review highlights the need to conduct additional well-structured research and studies addressing the complexity of factors related to peri-implant health. Such research is essential for the development of even more effective and personalized therapeutic approaches aimed at meeting the specific needs of each patient.
