No âmbito da cirurgia regeneradora os concentrados plaquetários estão relacionados com numerosos procedimentos cirúrgicos.
O potencial terapêutico baseia-se na libertação de fatores de crescimento plaquetários, todavia outras componentes como leucócitos e fibrina parecem ter importância no processo de cura.
O objetivo deste trabalho foi o de analisar os protocolos que tentam incluir, cada vez mais, leucócitos e fibrina (L-PRP, L-PRF, A-PRF) para comparar a maior eficácia biológica e cirúrgica entre os preparados de primeira e segunda geração
A análise comparativa foi focada no potencial antimicrobiano, na regeneração tecidular e na prática cirúrgica diária, tendo sido analisados estudos in vitro e in vivo.
Resultados heterogéneos foram evidenciados no impacto produzido pelos leucócitos, por outro lado, parece esclarecido que a membrana de fibrina presente nos novos concentrados melhora as propriedades biológicas e facilita a aplicação clínica do biomaterial. São necessários, futuramente, mais estudos clínicos in vivo.
Platelet concentrates are related to numerous surgical procedures in regenerative surgery. The therapeutic potential is based on the release of platelet growth factors, but other components such as leukocytes and fibrin appear to be important in the healing process. The objective of this study was to analyze the protocols that tried to include more leukocytes and fibrin (L-PRP, L-PRF, A-PRF) to compare the greater biological and surgical efficacy between the preparations of the first and second generation Comparative analysis was focused on antimicrobial potential, tissue regeneration and daily surgical practice, analyzing studies in vitro and in vivo Heterogeneous results were showed in the impact produced by the leukocytes, on the other hand, it seems clear that the fibrin membrane present in the new concentrates improves the biological properties and facilitates the clinical application of the biomaterial. In the future more clinical studies are needed.
