A Doença periodontal é uma patologia oral que se pode apresentar de duas
formas: gengivite e periodontite. A gengivite é a forma mais ligeira e reversível da
doença enquanto que a periodontite afecta irreversivelmente o osso alveolar e o
ligamento periodontal. Ambas possuem uma etiologia multifactorial e a sua progressão
depende da alteração do microbioma oral, principalmente da composição da placa
bacteriana. As espécies periodontais Porphyromonas gingivalis, Prevotella intermedia e
Fusobacterium nucleatum estão associadas com o desenvolvimento de cancro oral, em
particular o carcinoma oral de células escamosas. Estas possuem mecanismos de
virulência agressivos induzindo citoquinas inflamatórias, inibindo a apoptose,
promovendo a invasão celular e induzindo alterações genómicas das células da mucosa
oral do hospedeiro. Este quadro inflamatório associado a fatores de risco, como o tabaco
e álcool proporciona um ambiente favorável ao desenvolvimento de cancro oral,
destacando-se a importância do médico dentista no diagnóstico precoce e controlo
destas patologias.
Periodontal disease is an oral pathology that can present in two ways: gingivitis and periodontitis. Gingivitis is the mildest and most reversible form of the disease, while periodontitis irreversibly affects the alveolar bone and periodontal ligament. Both have a multifactorial etiology and their progression depends on the alteration of the oral microbioma, mainly the composition of the plaque. The periodontal species Porphyromonas gingivalis, Prevotella intermedia and Fusobacterium nucleatum are associated with the development of oral cancer, in particular oral squamous cell carcinoma. These have aggressive virulence mechanisms inducing inflammatory cytokines, inhibiting apoptosis, promoting cell invasion and inducing genomic alterations of the host oral mucosa cells. This inflammatory condition associated with risk factors such as tobacco and alcohol provides a favourable environment for the development of oral cancer, highlighting the importance of the dentist in the early diagnosis and control of these pathologies.
