As doenças periodontais necrosantes são um grupo de doenças infeciosas caracterizadas por um processo inflamatório agudo e pela presença de destruição periodontal. De causa multifatorial, afetam preferencialmente os adolescentes e adultos jovens. Representam uma condição séria, porém rara, mais comum em países subdesenvolvidos e áreas com má nutrição e más condições de vida. O diagnóstico é baseado no exame clínico e radiográfico.
Este trabalho expõe o caso de uma paciente de 19 anos, com diagnóstico compatível com Gengivite Necrosante. A paciente apresentava necrose limitada à margem gengival e papila interdentária, com sintomatologia dolorosa, hemorragia gengival ao toque, pseudomembrana, placa bacteriana visível, halitose e mal-estar generalizado. Durante a anamnese, a paciente relatou ser saudável, confidenciando um período de elevado stresse emocional. Neste caso, foi prescrito antibiótico, AINEs e clorhexidina e foram dadas instruções de higiene oral e realizados procedimentos de instrumentação mecânica. A paciente foi seguida de Outubro de 2022 até Maio de 2023 com resolução da doença.
Necrotizing periodontal diseases are a group of infectious diseases characterized by an acute inflammatory process and the presence of periodontal destruction. With multifactorial cause, it affects preferably teenagers and young adults. It is a serious but rare condition, more common in underdeveloped countries and areas with poor nutrition and poor living conditions. Diagnosis is based on clinical and radiographic examination. This study exposes the case of a 19-year-old patient with diagnosis compatible with Necrotizing Gingivitis. The patient presented gingival necroses limited to the gingival margin and interdental papilla, with painful symptoms, gingival bleeding to the touch, pseudomembrane, visible bacterial plaque, halitosis and generalized malaise. During the anamnesis, the patient reported that she was healthy, confiding a period of high emotional stress. In this case, antibiotics, NSAIDs and chlorhexidine were prescribed and oral hygiene instructions and mechanical instrumentation procedures were performed. The patient was followed from October 2022 to May 2023 with resolution of the disease.
