Dual-Energy X-ray Absorptiometry (DEXA) is widely regarded as a reference method for body composition assessment. However, its routine use is limited by cost, availability, and the need for specialized equipment. The present study aimed to assess the association of Adductor Pollicis Muscle Thickness (APMT), Mid-upper Arm Muscle Circumference (MAMC), and Leg Muscle Circumference (LMC) with Appendicular Lean Soft Tissue Mass (ALST) estimated by DEXA. A cross-sectional observational study was carried out based on data collected as part of the NutriFunction project, including 778 community-dwelling adults, of whom 523 were women and 255 were men. Anthropometric measurements were collected according to standardized procedures, and ALST was obtained using DEXA. The association between the anthropometric indicators and ALST tertiles was analyzed using multinomial logistic regression, adjusted for age, height, and total fat mass. Positive associations were observed between the anthropometric indicators and the higher ALST tertiles in both sexes. MAMC showed the strongest associations with the third ALST tertile (OR = 2.62 in women and OR = 3.04 in men; both p < 0.001), followed by LMC (OR = 2.41 and OR = 2.68, respectively; both p < 0.001). APMT was significantly associated only with the third ALST tertile, suggesting a less consistent association with appendicular muscle mass. These findings suggest that MAMC and LMC are promising anthropometric indicators of muscle mass, whereas APMT demonstrated a less consistent association with ALST. These results reinforce the potential of anthropometry as a complementary approach for the indirect assessment of muscle mass in settings where instrumental methods are not available.
