À medida que a doença progride o organismo torna-se mais suscetível a infecções oportunistas. Existe uma relação entre a aparição de lesões orais com a contagem de linfócitos TCD4, assim como podem surgir alterações provocadas pela terapia anti-retroviral (HAART). Como tal, as lesões orais são marcadores vitais da infecção viral e podem ser indicativas de seroconversão aguda e/ou indicar progressão da doença e imunossupressão acentuada em indivíduos infetados. As lesões orais mais frequentemente diagnosticadas incluem a Candidíase, Queilite angular, a Leucoplasia Pilosa, Sarcoma de Kaposi, Linfoma non Hodgkin e Doenças Periodontais Ulcerativas.
